LIABILITY RELEASE

I am medically, physically, emotionally and in all respects fit and able to participate in these classes. I have been examined by a licensed physician within the last six months and have been found by such physician to be in good physical health and fully able to perform all exercises which I'm to learn and perform during my time with Alyssa Kuzins LLC. I will faithfully follow all instructions given me by them and as to when, where and how to perform and not to perform movement postures such as dance or yoga, it being understood that any deviation by me from such instructions shall be at my own risk. I will not hold Alyssa Kuzins LLC responsible for any injuries suffered by me caused in whole or in part by my failure to faithfully follow instructions, avoid activity that I know will not work for my body, or by any physical impairment of mine not fully disclosed to them in writing. 


In consideration of being permitted to participate in these classes, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program. I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against Alyssa Kuzins LLC, my heirs or legal representatives, forever release, waive, discharge and covenant negligence or other acts. 
I agree I will be fully and financially responsible for my own physical condition and well being during these classes and will follow the safety precautions and instructions prescribed by Alyssa Kuzins LLC and any hired operators.


I will not hold Alyssa Kuzins LLC responsible for any pain, discomfort, or emotional distress I experience during or after the retreat.

I understand that the activities offered are not a substitute for medical care or therapy. I understand that Alyssa Kuzins LLC is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness. I also understand that yoga and meditation are not religious teachings.

I have read carefully and understand this agreement with Alyssa Kuzins LLC and/ or its affiliated organizations.
